JOB DESCRIPTION: Case Manager/Housing Navigator
POSITION TITLE: Case Manager/Housing Navigator
DEPARTMENT: Rapid Re-Housing
FLSA: Full-time, Non-Exempt
PAY RANGE: $18-$23 an hour, commensurate with experience and education
Job Description: Case Managers/Housing Navigators are responsible for case management, housing navigation and location as needed, housing assistance, and assistance accessing support services related to retaining permanent housing and ongoing self-care.
Responsibilities:
- Provide case management services for individuals/households;
- Complete assessments on each client and complete VI-SPDAT and enter information into HMIS/CES;
- Assist clients in completing housing applications and move in, if needed;
- Assist and locate affordable housing, if needed;
- Landlord mediation;
- Provide ongoing support and case management to ensure retention including accessing benefits and coordinating, brokering, and scheduling appropriate client services with external medical, dental, legal, housing, counseling, vocational training, continued education, social/recreational activities to meet client's needs and treatment goals;
- Assist with credit repair, and job search if needed;
- Communicate and adhere to program policies and procedures and model professional rapport to all clients in accordance with the Operations Manual, including strict privacy and security protocols to protect personal information (PI);
- Assist/maintain appropriate documentation regarding client's progress, services received and special incidents in individual resident case files;
- Complete and submit time-sensitive reports to referring entities and/or agents;
- Ensure compliance with organization, contractual, local/state/federal mandates, including confidentiality laws;
- Perform other duties as assigned by the Program Manager/Director.
Qualifications/Requirements: Case Managers/Housing Navigator will possess these minimum qualifications:
- 2-3 years experience in Case Management working with vulnerable populations and/or mental health settings;
- Have reliable transportation, position travels countywide;
- BA/BS in Social Work or comparable discipline preferred;
- Knowledge of Microsoft Office (Outlook, Word, Excel);
- HMIS, CES, and CIE knowledge, preferred;
- Ability to work independently and manage time effectively;
- Strong organizational skills;
- Bilingual speakers encouraged;
- Voucher experience encouraged;
- Must have reliable transportation;
